U.S. IRS Form 8937
Under U.S. federal income tax law, issuers are required to report on Form 8937 the quantitative change in the basis of a security resulting from an organizational action. This requirement therefore applies to the exchange of TELUS Non-Voting shares for TELUS Common shares that occurred on February 4, 2013 and the two-for-one stock splits of the TELUS Common shares that were effective April 16, 2013 and March 13, 2020. The reporting requirement was put in place to allow investors and others to accurately adjust a shareholder’s tax basis over time. TELUS does not provide tax advice. TELUS reservation application for common shares
This form is to be completed for all transfers of TELUS Common Shares to non-Canadian shareholders, as defined in the Corporation’s Articles and the ownership and control regulations pursuant to the Telecommunications Act (Canada). The form is normally completed by the financial institution who is executing a purchase order for a non-Canadian client. The reservation system requires non-Canadian purchasers of common shares to obtain a reservation number from Computershare.
