How to use

How to use

  1. Submit form here. Please allow us to process the information based on our workload capacity.
  2. One of our team members will reach out to you to clarify requirements. We will be discussing the following:
    • The contents of the survey (Survey template copy deck)
    • If you are collecting PII that we don't currently own, please review the Artifact Records to find the completed master PIA and SBDR as reference,

    • Where the survey will live in
    • The implementation strategy
    • The data visualization requirements (i.e. Domo, Slack, Qualtrics)

NOTE: Implementation does not begin until 80% of the requirements are gathered.

Post implementation warranty

If you're planning on making changes to an existing survey, including:

  • changing the channel on how the survey is displayed (i.e. email, web, SMS)
  • changing a question and the question type
